Job Description

A social services organization in New York is currently seeking a Case Manager to join their team. This position involves 100% fieldwork across the Bronx and Manhattan boroughs.

About the Opportunity:
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm (35-hour work week)
Responsibilities:
  • Providing direct client services, including intake and assessment
  • Managing a caseload and documenting progress in APSNET and ETO
  • Monitoring risk and adapting service plans as needed
  • Referring clients to appropriate external services
  • Conducting home visits and telephonic communication weekly
Qualifications:
  • 1+ year of Case Management experience
  • Bachelor's Degree in Social Work and/or a related field
  • Proficiency in Microsoft Office and standard business technology
  • Solid assessment and documentation skills

Desired Skills:

  • 4+ years of Case Management experience
  • Bilingual (English and Spanish)
